ABSTRACT

ABSTRACT Purpose: To evaluate the quality of life of children with strabismus and understand their consequences in the functional and psychosocial aspects of the childhood. Methods: A semi-structured interview adapted of Adult Strabismus-20 questionnaire was conducted with parents or the legal responsible person of children aged between 3 and 13 years old attended at the public health care. A likert-scale of five points was used to measure the impact of strabismus in childhood, from psychosocial and functional aspects. Results: 97 children were included in the study. 61,4% of the children had esotropia (ET), while 35.9% had exotropia (XT). In each group, the magnitude of deviation was classified in smaller (<) or equal/greater (≥) then 30 prismatic diopters. When analyzing the functional and the psychosocial scores of the groups separated by the type of strabismus and magnitude of deviation, the average score of the group with esotropia and the patients with larger deviations were worse. The correlation value between the psychosocial and functional scores was 200.656 (p <0.01). Conclusion: Strabismus in childhood is associated with many negative effects. These consequences should be considered when deciding for surgical treatment on early ages. The strabismus treatment may change positively how these children perceive themselves, resulting in benefits on quality of life from both the functional and psychosocial perspectives.


RESUMO Objetivo: Avaliar a qualidade de vida de crianças com estrabismo e entender suas consequências em aspectos funcionais e psicossociais da infância. Métodos: Uma entrevista semiestruturada adaptada do questionário Adult Strabismus-20 foi realizada com os pais ou responsáveis legais das crianças entre 3 e 13 anos atendidas na rede pública de saúde. Uma escala do tipo Likert de cinco pontos foi usada para mensurar o impacto do estrabismo na infância, do ponto de vista funcional e psicossocial. Resultados: Foram incluídas no estudo 97 crianças, sendo 61,4% com esotropia e 35,9% com exotropia. Em cada grupo, a magnitude do desvio foi classificada em menor ou maior que 30 dioptrias prismáticas. Quando analisados os escores funcionais e psicossociais de cada grupo separados por tipo e magnitude do desvio, a média do escore do grupo com esotropia e dos pacientes com maior desvio foi pior. O valor de correlação entre os escores funcionais e psicossociais foi de 0,656 (p<0,01). Conclusão: Estrabismo na infância está associado a muitos efeitos negativos. As consequências devem ser levadas em consideração quando da decisão por tratamento cirúrgico em idades precoces. O tratamento do estrabismo pode alterar de forma positiva a percepção que a criança tem de si mesma, resultando em benefícios na qualidade de vida, tanto em aspectos funcionais quanto psicossociais.

Se efectuó un estudio longitudinal, prospectivo y comparativo de 43 adultos con estrabismo, quienes acudieron al Servicio de Oftalmología del Hospital Infantil Sur de Santiago de Cuba, desde enero del 2015 hasta mayo del 2017, a fin de valorar su autoestima. A todos se les realizó un examen oftalmológico completo, que mostró el grado de desviación media antes y después de la cirugía. Se les aplicó una prueba de autoestima de Rosenberg, previa a la intervención y 2 meses después de esta. Prevalecieron el sexo femenino, la edad media de 48,5 años y la exotropía. Antes de la cirugía, el nivel de autoestima estuvo por debajo de 25 puntos en la mayoría de los pacientes (81,3 por ciento); sin embargo, posterior a esta fue baja en solo 2,3 por ciento y normal en 90,6 por ciento, lo que causó un impacto favorable en la calidad de vida e integración social de los afectados.


A longitudinal, prospective and comparative study of 43 adults with strabismus who went to the Ophthalmology Service of the Southern Pediatric Hospital in Santiago de Cuba, from January, 2015 to May, 2017 was carried out, in order to evaluate their self-esteem. A complete ophthalmological exam was carried out to all, which showed the average deviation degree before and after surgery. A Rosenberg self-esteem test was applied, previous to the intervention and 2 months after it. The female sex, the mean age of 48.5 years and the exotropy prevailed. Before surgery, the self-esteem level was below 25 points in most of the patients (81.3 percent); however, after this it was low in only 2.3 percent and normal in 90.6 percent, what caused a favourable impact in the life quality and social integration of the affected patients.

Purpose: To evaluate the impact of strabismus on quality of life. Methods: This cross-sectional study included 101 individuals aged 7-67 years with strabismus. They were interviewed and made to answer a questionnaire with 20 questions intended to assess the individual's interaction with their social and working environment, sensory perception, and limitations in their daily lives. There were five types of possible responses for each question: never, rarely or very few times, sometimes, frequently, and always. The questionnaire was based on the AS-20 and contained 11 questions assessing psychosocial aspects and 9 questions assessing functional aspects. Among those who were interviewed, 24.8% had undergone surgical correction of strabismus. Results: We observed a significant impact of strabismus on the quality of life of the interviewed individuals. Feelings of sadness and inferiority because of strabismus were reported by 74.2% and 58.4% respondents, respectively. In terms of functionality, 12.1% reported difficulty in reading, 14% said they had difficulty in depth perception (stereopsis), and 17.8% frequently or always associated pain or burning sensation in the eyes to strabismus. A significant difference was detected in the quality of life scores for the psychosocial aspect among patients who had and had not undergone surgery (Wilcoxon test, 158; p<0.001). Individuals who had undergone surgery had a better quality of life from the psychosocial perspective. Conclusions: In this evaluation, we found a significant negative interference of strabismus on quality of life from both the functional and psychosocial perspectives. This demonstrated the importance of treatment for strabismus, regardless of age, because it can interfere with the functional well-being of the individual. .


Objetivo: Avaliar a interferência do estrabismo na qualidade de vida em indivíduos estrábicos. Métodos: Estudo transversal envolvendo 101 indivíduos estrábicos entre 7 e 67 anos. Eles foram entrevistados e responderam um questionário com 20 perguntas aplicadas pelos pesquisadores. As perguntas tinham como objetivo avaliar a interação do indivíduo com o seu meio social, de trabalho, percepção sensorial e limitações na sua vida diária, com cinco tipos de respostas para cada pergunta: nunca, raramente ou muito pouco, algumas vezes, frequentemente e sempre. O questionário foi baseado no AS-20 e contendo 11 questões avaliando aspectos psicossociais e 9 questões avaliando aspectos funcionais. Entre os indivíduos entrevistados, 24,8% haviam sido submetidos a correção cirúrgica do estrabismo. Resultados: Observou-se impacto significativo do estrabismo sobre a qualidade de vida dos indivíduos avaliados. Sentir-se incomodado ou inferiorizado em decorrência do estrabismo foram relatados por 74,2% e 58,4% dos entrevistados respectivamente. No quesito de funcionalidade, 12,1% relataram dificuldade para ler, 14% disseram ter dificuldade na percepção de profundidade (estereopsia) e 17,8% associaram dor ou ardor nos olhos ao estrabismo frequentemente ou sempre. Diferença significativa foi detectada com respeito ao escore de qualidade de vida no aspecto psicossocial entre indivíduos que realizaram e não realizaram cirurgia (teste de Wilcoxon = 158, p-valor <0,001). Indivíduos que realizaram cirurgia possuíam melhor qualidade de vida no aspecto psicossocial. Conclusões: Nessa avaliação, percebe-se uma interferência negativa muito significativa do estrabismo na qualidade de vida, tanto funcional como psicossocial nos indivíduos pesquisados. ABSTRACT

The aim of the study was to know the response of the relatives attending the squint surgery of their ward. A trained secretary administered an eight item questionnaire by live / telephonic interview. Of the 44 attendees, two left the Operation Theater before completion of the surgery. Mean age of the patients was 7.2 years ± 7.8 and that of the attendees was 36.1 years ± 8.5. Forty patients had a surgery under general anesthesia and four under local anesthesia. Eleven (25%) attendees experienced an increase in anxiety. Thirty-six (82%) attendees reported increased transparency, 38 (86%) reported increased confidence, and 43 (98%) reported increased awareness. None found any disadvantage. Twenty-seven (61%) recommended this practice for all and 16 (36%) recommended the practice selectively. The internal validity of the questionnaire was fair (Cronbach's Alpha = 0.6). It was concluded that the presence of relatives in the Operation Theater during the surgery could bring in more transparency, accountability, confidence, awareness, and trust.

ABSTRACT

Este trabajo refleja el sentido de lo bello en la práctica médica, especialmente en el paciente con estrabismo disociado y la importancia de la formación de adecuados valores estéticos en consonancia con el alto valor humanista que la sociedad promueve. En tal sentido los valores estéticos tienen también un razonable lugar en la vida del ser humano, cómo colaborar en la formación y comprensión de estos es una tarea que toca también a los profesionales de la salud. Pretendemos acercarnos a las diferentes interpretaciones y a la identificación de esta categoría filosófica relacionándolo con la corrección quirúrgica de esta patología que favorece una mejora no sólo la apariencia estética, sino también la función psicosocial y la calidad de vida


This paper deals with the concept of beauty in relation to the medical practice, especially in patients with dissociated strabismus, as well as the importance to develop the right aesthetics values pursuant to the high humanistic values of the society that promotes them. In this regard, the esthetic values also have an important place in the life of human beings, so how to cooperate in the formation and understanding of these values should be a mission of every health professional. This paper was intended to approach different interpretations, as well as the identification of this philosophical category by relating it to the surgical correction of this pathology that favors improvement not only in the esthetic appearance but also in the psychosocial function and the quality of life of the patients
